Intro

The goal of the Health Outcomes Questionnaire is to assess the total public health benefit of the work that an organization is doing. The Health Outcomes Questionnaire assesses outcomes in terms of YLLs or DALYs:

Year of Life Lost (YLL)
A YLL is a unit of measurement for comparing health outcomes. YLLs are similar to DALYs, although they don't account for disability caused by disease.

Disability-Adjusted Life Year (DALY)
A DALY is a unit of measurement that's widely-used when trying to measure health outcomes. DALYs are measured by taking into account the years of life gained and the years that are lived with some disability, with a weighting for the severity of the disability. For example, if a 35-year-old smoker quits smoking, they might gain 8 DALYs. A new national tax on cigarettes in some country might save 12,000,000 DALYs. Health Outcomes Questionnaire Template

For most organizations, it's very difficult to estimate the total impact of the organization's work in terms of YLLs or DALYs. The Health Outcomes Questionnaire relies on educated guesses and estimates found in books like Global Crises, Global Solutions.

Most of our other Questionnaires have templates that include simple tables that can be filled in by looking up basic facts. In contrast, the Health Outcomes Questionnaire is more of an "essay test" answer, requiring some research and guesswork.

The Health Outcomes Questionnaire Template is just a blank page, and it's up to you to figure out what to fill in.
